If you don&#8217;t have your ID Card you have to know your ID Nunber and your insurance company name.<\/li>\n\n\n\n<li>If you are the patient by the insurance belongs to your relatives we need to know ID number and date of birth for the primary insured person.<\/li>\n\n\n\n<li> If you have more than one insurance coverage we will be glad to file claims to both insurances.
